Families who use Harbor Medical for Sports Physicals

Please use the following template for your child's sports physical.  This template is issued by the Massachusetts Interscholastic Athletic Association (MIAA) and fulfills the criteria for an acceptable physical exam to play sports at Silver Lake Regional High School.
